MedCentral Home Care

We offer cost-effective health care by our highly qualified professionals for patients who no longer need 24-hour supervision but require some type of skilled follow-up care. Home Care Services are available on call 24 hours a day, seven days a week, including holidays and weekends. Patients must be under the care of a physician.

We offer skilled nursing, physical therapy, occupational therapy, speech therapy, medical social workers and home health aides. The professional health care team also includes certified oncology and wound care specialists.

Home care focuses on educating patients and their caregivers so they are more comfortable managing their condition at home. Through home care, re-hospitalization and exacerbations decrease, along with health care costs.

We are Medicare and Medicaid certified and a member of both the Ohio Council for Home Care and the National Association for Home Care. A physician's referral is required. Call MedCentral Home Care at (419) 526-8442 for help in securing a referral.
