  To send an e-mail to a patient at one of our hospitals, complete the form below and click "submit." Messages will be printed out and taken by hand to patients once each weekday morning, excluding holidays. E-mail on weekends or after hours will be delivered the morning of the next business day.

We cannot deliver messages received after a patient has been discharged. For patient information, or for urgent messages, please call 419-526-8350.

Because e-mail messages sent via the web are not encrypted, it's possible that an unauthorized person could intercept messages. We cannot be responsible for privacy of information sent to us via e-mail.
